How to use it
- Choose "Blank to print" for a paper form, or "Filled in" to enter the client's details here.
- Enter your studio, artist and appointment details; they appear on both versions.
- For a filled form, add the client, ID check, design and health answers, and tick the acknowledgements.
- Download the PDF or click Print, then have the client (and guardian if needed) sign it on paper.
How it works
The form has sections for the studio and appointment, the client's details and ID check, the design and placement, twelve health questions with Yes/No columns, acknowledgements to tick or initial, a short aftercare summary and signature boxes for the client and artist. The blank version also includes a parent or guardian line; on a filled form it appears only when the client is under the minimum age you enter.
The age is calculated from the date of birth and the appointment date (or today). The minimum age setting only triggers a warning: the legal rules are not built in because they vary so much. In some places tattooing anyone under 18 is prohibited even with parental consent, in others a guardian may consent from a certain age.
The PDF is created in your browser with embedded Noto Sans fonts, A4 size, with page numbers. It is a template: it is not legal advice and has not been reviewed for any jurisdiction. Health questions do not replace medical advice.
Examples
Stack of blank forms for the front desk
Enter only the studio name, address and licence number, choose Blank to print and print as many copies as you need.
Filled form on a tablet
Fill in the client's details, design and health answers together at the counter, download the PDF and print it for signatures.
Checking a young client
Enter the date of birth and your local minimum age; the tool warns you when the client is under it and adds a guardian signature line.
Frequently asked questions
Is this form legally valid?
It is a general template, not legal advice. Requirements differ by country and region (record keeping, wording, minimum age, health checks). Have a local professional or your licensing authority review it.
How old do you have to be to get a tattoo?
It depends on where you are. Many places set 18 as the minimum; some allow younger clients with a parent or guardian's written consent, and some forbid tattooing minors entirely. Check your local law.
Where are the client's details stored?
Nowhere. The form exists only in your browser tab until you download or print it. Store signed paper or PDF forms securely according to your local data protection rules.
Can I add my own questions?
Use the health notes field, or download the blank version and add questions by hand. The printed PDF cannot be edited directly here.
